Electronic Financial Forum Subscription Form

For an annual fee of $3,000 a company is eligible for one line into the Forum teleconference each month. Forum subscription fees are payable annually prior to the start of October each year. The Forum subscription year runs from October 1 to September 30 of the following year. To subscribe to the Forum, participants should submit their subscription applications along with payment via mail or electronically. Those who submit subscriptions after the subscription year has commenced will have their membership fee prorated and the remainder of the fee applied to the up coming year and will be billed for the balance later that year. If you are submitting a check via mail please make checks payable to the "Electronic Financial Services Council" and send with the printed application form to:
